Reasoned debate around the issue of the need to abolish work records has been going on for a long time. Nevertheless, things are still there: the book is still the main document about production activities and employee experience. The article contains instructions for filling out work books 2020 with examples.

Employer Responsibilities

All employers (except for individuals who are not individual entrepreneurs), in accordance with Article 66 of the Labor Code of the Russian Federation, are required to keep records for each employee who has worked for them for more than five days, if this activity is the main activity for the employee.

Where to buy a form

The employee does not need to independent purchase form. The employer, in accordance with paragraph 44, is obliged to constantly have the required number of forms and forms available. Instructions for maintaining work records (2020): legislative framework

Explain and regulate the administration of the Rules approved. They contain general questions the procedure for maintaining, storing, making changes, corrections, issuing, including books.

The rules consist of eight sections:

  1. General provisions.

Each section of the instructions for filling out the work book 2020 contains relevant instructions for each of the areas. The procedure for making entries in work books is established, which contains seven sections.

General provisions

Entries must be neat, in black, blue or purple ink, without any abbreviations. For example, the surname, first name and patronymic must be written in full; writing a surname with initials is not allowed. Arabic numerals are used. Columns are filled in capital letters. There is only one exception: when the employee signs the title page You can write your full name in block letters to avoid confusion and possible questions.

Filling out employee information

When filling out a work book for the first time, the following data must be indicated on the title page:

  • surname, name, patronymic of the employee;
  • full date of birth;
  • information about the education received;
  • specialist diploma (if available).

The section contains information on the basis of which documents and in what order information about the owner is entered, including when changes are made to personal data.

Features of filling out information about dismissal (termination of an employment contract) and hiring (appointment) in connection with the transfer of an employee to another permanent job with another employer (to another organization) or his transfer to an elective job (position)

When an employee moves from one position to another, the corresponding mark must be made in the work book. It is important to note that the transfer is considered legal only if there is the consent of the employee himself.

There are two types of transfers from one position to another:

  • internal, while the employer remains the same;
  • external, the contract with the current employer is terminated and a new employment contract is concluded.

Also, the transfer can be permanent or temporary, for example when replacing a temporarily absent employee. In this case, in the third column:

  • when transferring externally, indicate the date and number of the order of dismissal from one organization (full name), the date and number of the order of enrollment in another organization (full name). Indication of positions is necessary in both cases;
  • when transferring internally, it is written as follows: “transferred to position” indicating it, the name of the department, if necessary.

Features of filling out a duplicate work book

In fact, the provisions of this section are divided into two subparagraphs: filling out a duplicate with the previous employer and filling out a duplicate if the employee worked for another employer before joining the organization.

Instructions for filling out work books 2020 also contain a form of a receipt and expenditure book for recording work book forms and inserts in them and a form for the Book of Accounting for the movement of work books and inserts in them.

The instructions for filling out the work book states that for all part-time workers, the corresponding entries in the book are made at their request. For external part-time workers on the basis of a document confirming part-time work (a certified copy of the employment order).

If an external part-time worker has not been fired from his main place of work and asks you to make an entry about part-time work, he should politely refuse, referring to the requirements of which all entries are made at the main place of work.

What if a part-time worker is fired from his main job?

From the point of view of legislation, any entry is made at the main place of business; this provision cannot be circumvented.

If the employer decides to change the terms of the contract for an external part-time worker and has drawn up a contract with him at the main place of work, the entry is made as usual.

Insert design

If all blank pages of the work are completed, the subsequent information is entered on the insert form. As a rule, it is sewn into the end of the document. And on the title page of the main book a stamp is placed indicating the issue of the insert, indicating its number and series. The cost of the insert is paid by the employee. The rules for filling out are the same as for the main labor document:

  1. The insert is filled out by the employer.
  2. The date is stamped on the title page.
  3. Up-to-date information concerning the worker is added.
  4. The entry is certified by the personal signature of the person filling it out and a seal.

If the employing organization has been renamed

Changing the name of the organization does not entail a change in the employee’s working conditions. But in accordance with paragraph 2.2 of the Instructions, this information must be entered in the work book. The filling procedure is as follows:

  • column 1 and column 2 are not filled in;
  • Column 3 - information about changes in the name of the organization is entered, the previous and current names are indicated in full and abbreviated forms;
  • Column 4 - indicate the date and details of the basis document.

INSTRUCTIONS

FOR COMPLETING WORK BOOKS

This Instruction in accordance with paragraph 13 of the Rules for maintaining and storing work books, producing work book forms and providing them to employers (hereinafter referred to as the Rules for maintaining work books), approved by Decree of the Government of the Russian Federation of April 16, 2003 N 225 “On work books”, establishes the procedure for filling out work books, inserts in them, duplicates of work books (hereinafter referred to as work books).

1.1. Entries of dates in all sections of work books are made in Arabic numerals (day and month - two digits, year - four digits). For example, if an employee was hired on September 5, 2003, an entry is made in the work book: “09/05/2003”. Entries are made carefully, with a fountain or gel pen, rollerball pen (including ballpoint), light-resistant ink (paste, gel) black, blue or purple and without any abbreviations. For example, it is not allowed to write "pr." instead of "order", "dispatch." instead of "order", "trans." instead of "translated" etc.
1.2. In the sections “Information about work” and “Information about awards” of the work book, crossing out previously made inaccurate, incorrect or other invalid entries is not allowed. For example, if it is necessary to change a specific entry about employment in the section “Information about work” after the corresponding last in this section of the entry, the subsequent serial number and the date the entry was made are indicated; in column 3, an entry is made: “The entry with the number such and such is invalid.” After this, the correct entry is made: “Accepted in such and such a profession (position)” and in column 4 the date and number of the order (instruction) or other decision of the employer, the entry from which was incorrectly entered into the work book, is repeated, or the date and number of the order is indicated ( order) or other decision of the employer, on the basis of which the correct entry is made. In the same manner, an entry on dismissal, transfer to another permanent job if the dismissal or transfer is recognized as illegal by the employer, the control and supervisory body, the labor dispute resolution body or the court and reinstatement to the previous job or a change in the wording of the reason for dismissal. For example: “The entry for number so-and-so is invalid, reinstated to previous job.” When changing the wording of the reason for dismissal, an entry is made: “The entry for number such and such is invalid, dismissed (indicated new wording)". Column 4 makes reference to the order (instruction) or other decision of the employer on reinstatement or changing the wording of the reason for dismissal. If there is an entry in the work book about dismissal or transfer to another permanent job, which is subsequently declared invalid, upon the written application of the employee a duplicate of the work book is issued without making an entry in it that has been declared invalid. At the same time, in the upper right corner of the first page of the duplicate work book the following inscription is made: “Duplicate” On the first page (title page) of the previous work book it is written: “A duplicate has been issued in return” with the indication. its series and numbers.

2. Filling out information about the employee

2.1. The information about the employee provided for by the Rules for maintaining labor books, indicated on the first page (title page) of the labor books, is filled out as follows: the last name, first name and patronymic are indicated in full, without abbreviating or replacing the first name and patronymic with initials, the date of birth is written down in full (day, month, year) on the basis of a passport or other identification document (for example, a military ID, foreign passport, driver’s license, etc.); a record of education (basic general, secondary general, primary vocational, secondary vocational, higher professional and postgraduate vocational) is carried out only on the basis of duly certified documents (certificate, license, diploma, etc.); a record of incomplete education at the appropriate level can be made on the basis of submitted duly certified documents (student card, grade book, certificate educational institution etc.); profession and/or specialty are indicated on the basis of documents on education, qualifications or the presence of special knowledge (when applying for a job that requires special knowledge or special training) or other properly executed documents.
2.2. After indicating the date of filling out the work book, the employee certifies the correctness of the entered information with his signature on the first page (title page) of the work book. The first page (title page) of the work book is also signed by the person responsible for issuing work books, after which the organization’s seal is affixed (stamp of the personnel service ) (if there is a seal), in which the work book was first filled out.
2.3. Changes to entries in work books about last name, first name, patronymic and date of birth are made on the basis of a passport, birth certificate, marriage certificate, divorce certificate, change of last name, first name, patronymic and other documents and with reference to their number and date. changes are made to the first page (title page) of the work book. The previous surname or first name, patronymic, date of birth are crossed out with one line and new data is recorded. Links to the relevant documents are made on the inside cover of the work book and certified by the signature of the employer or a person specially authorized by him and the seal of the organization (or the seal of the personnel service) (if there is a seal).
2.4. Changes (additions) on the first page (title page) of the work book of entries about new education, profession, specialty obtained are carried out by adding existing entries (if they already exist) or filling out the corresponding lines without crossing out previously made entries.

3. Filling out job information

3.1. In column 3 of the “Work Information” section of the work book, the full name of the organization is indicated as a heading, as well as the abbreviated name of the organization (if available). Under this heading in column 1 is the serial number of the entry being made, in column 2 the date of employment is indicated .In column 3, an entry is made about acceptance or appointment in structural unit organization, indicating its specific name (if the condition of working in a specific structural unit is included in the employment contract as essential), the name of the position (job), specialty, profession indicating qualifications, and in column 4 the date and number of the order (instruction) or another decision of the employer, according to which the employee was hired. Records of the name of a position (job), specialty, profession indicating qualifications are made, as a rule, in accordance with the organization’s staffing table. If, in accordance with federal laws, the performance of work in certain positions, specialties or professions is associated with the provision of benefits or the presence of restrictions, then the name of these positions, specialties or professions and qualification requirements they must comply with the names and requirements provided for in the relevant qualification reference books. The establishment of a second and subsequent profession, specialty or other qualification for an employee is noted in the work book indicating the ranks, classes or other categories of these professions, specialties or skill levels. In this case, in the work book in column 1 of the section “Information about work” the serial number of the entry is indicated, in column 2 the date of establishment of the second profession is indicated, in column 3 the entry is made: “The second profession “Electric and gas welder” has been established with the assignment of 3rd category”, in column 4 indicates the relevant certificate, its number and date. At the request of the employee, information about part-time work is recorded in the work book at the place of main work on the basis of a document confirming part-time work. In column 1 of the “Work Information” section of the work book, the serial number of the entry is entered, in column 2 the date of employment as a part-time worker is indicated, in column 3 an entry is made about acceptance or appointment as a part-time worker in a structural unit of the organization, indicating its specific name ( if the condition of working in a specific structural unit is included in the employment contract as an essential one), the name of the position, specialty, profession indicating qualifications, column 4 indicates the name of the document on the basis of which the entry was made, with reference to its date and number. A record of dismissal from this job is made in the same manner.
3.2. If during the employee’s work the name of the organization changes, then an entry about this is made in a separate line in column 3 of the “Information about work” section of the work book: “Organization such and such has been renamed to such and such date,” and in column 4 it is entered the basis for renaming is an order (instruction) or other decision of the employer, its date and number.
3.3. In the work books of persons who have served a sentence of correctional labor, an entry about the non-inclusion of work time during the period of serving the sentence in the continuous work experience is entered as follows. In the “Information about work” section of the work book, in column 1 the serial number of the entry is entered, in column 2 - the date of entry, in column 3 the entry is made: “Work time from such and such a date (day, month, year) to such and such date (day, month, year) is not counted towards continuous work experience.” Column 4 indicates the basis for making an entry in the work book - an order (instruction) or other decision of the employer (issued in accordance with the court verdict), its date and number.
3.4. When continuous work experience is restored in accordance with the established procedure, the following entry is made in the employee’s work book at the last place of work in column 3 of the section “Information about work”: “Continuous work experience was restored from such and such a date, month, year”, in column 4 a reference is made to the corresponding name of the document on the basis of which the entry was made, with reference to its date and number.

4. Filling out information about the award

The procedure for entering information about awards is as follows: in column 3 of the “Information about awards” section of the work book, the full name of the organization is indicated as a heading, as well as the abbreviated name of the organization (if any); below in column 1 the serial number of the entry is entered (numbering increasing throughout the entire period labor activity employee); Column 2 indicates the date of award; Column 3 records who awarded the employee, for what achievements and with what award; Column 4 indicates the name of the document on the basis of which the entry was made, with reference to its date and number.

5. Filling out information about dismissal (termination of employment contract)

5.1. An entry about dismissal (termination of an employment contract) in the employee’s work book is made in the following order: in column 1 the serial number of the entry is entered; Column 2 indicates the date of dismissal (termination of the employment contract); in column 3 an entry is made about the reason for dismissal (termination of the employment contract); column 4 indicates the name of the document on the basis of which the entry was made - an order (instruction) or other decision of the employer, its date and number. The date of dismissal (termination of the employment contract) is considered the last day of work, unless otherwise established by federal law, the employment contract or agreement between the employer and the employee. For example, when an employment contract with an employee is terminated due to a reduction in staff, October 10, 2003 is determined to be the last day of his work. The following entry must be made in the employee’s work book: in column 1 of the “Work Information” section, the serial number of the entry is entered, in column 2 the date of dismissal is indicated (10.10.2003), in column 3 the entry is made: “Dismissed due to a reduction in the number of employees of the organization, paragraph 2 of article 81 Labor Code Russian Federation", column 4 indicates the date and number of the order (instruction) or other decision of the employer on dismissal.
5.2. Upon termination of an employment contract on the grounds provided for in Article 77 of the Labor Code of the Russian Federation (except for cases of termination of the employment contract at the initiative of the employer and due to circumstances beyond the control of the parties (clauses 4 and 10 of this article), an entry about dismissal is made in the work book ( termination of the employment contract) with reference to the corresponding paragraph of this article. For example: “Dismissed by agreement of the parties, paragraph 1 of Article 77 of the Labor Code of the Russian Federation” or “Dismissed by at will, paragraph 3 of Article 77 of the Labor Code of the Russian Federation."
5.3. When terminating an employment contract at the initiative of the employer, a record of dismissal (termination of the employment contract) is made in the work book with reference to the relevant paragraph of Article 81 of the Labor Code of the Russian Federation or other grounds for terminating the employment contract at the initiative of the employer, provided for by law. For example: “Dismissed due to liquidation of the organization, paragraph 1 of Article 81 of the Labor Code of the Russian Federation" or "Dismissed due to termination of access to state secrets, paragraph 12 of Article 81 of the Labor Code of the Russian Federation."
5.4. When an employment contract is terminated due to circumstances beyond the control of the parties, an entry is made in the work book about the grounds for termination of the employment contract with reference to the relevant paragraph of Article 83 of the Labor Code of the Russian Federation. For example: “Dismissed due to non-election to a position, paragraph 3 of Article 83 Labor Code of the Russian Federation" or "The employment contract was terminated due to the death of the employee, paragraph 6 of Article 83 of the Labor Code of the Russian Federation."
5.5. When an employment contract is terminated on additional grounds provided for by the Labor Code of the Russian Federation or other federal laws, records of dismissal (termination of the employment contract) are made in the work book with reference to the relevant article of the Labor Code of the Russian Federation or other federal law. For example: “Dismissed due to with repeated gross violation of the charter of the educational institution, paragraph 1 of Article 336 of the Labor Code of the Russian Federation within a year" or "Dismissed due to reaching the age limit established for holding a public position civil service, paragraph 2 (1) of article 25 Federal Law dated July 31, 1995 N 119-FZ “On the fundamentals of the civil service of the Russian Federation.”
5.6. When an employment contract is terminated at the initiative of an employee for reasons that are associated by law with the provision of certain benefits and benefits, a record of dismissal (termination of the employment contract) is entered into the work record book indicating these reasons. For example: “Dismissed of her own free will due to the transfer of her husband to work in another locality, paragraph 3 of Article 77 of the Labor Code of the Russian Federation” or “Dismissed of her own free will due to the need to care for a child under 14 years of age, paragraph 3 of the article 77 of the Labor Code of the Russian Federation."

6. Features of filling out information about dismissal (termination of an employment contract) and hiring (appointment) in connection with the transfer of an employee to another permanent job with another employer (to another organization) or his transfer to an elective job (position)

6.1. Upon dismissal (termination of an employment contract) in connection with the transfer of an employee to another permanent job with another employer (another organization), in column 3 of the “Information about work” section of the work book, it is indicated in what order the transfer is carried out: at the request of the employee or with his consent .When hired to a new place of work, an entry is made in the employee’s work book in column 3 of the “Information about work” section, provided for in paragraph 3.1 of these Instructions, indicating that the employee was accepted (appointed) in the order of transfer.
6.2. Upon dismissal (termination of an employment contract) in connection with the transfer of an employee to an elective job (position) to another employer (to another organization), an entry is made in the work book: “Dismissed in connection with the transfer to an elective job (position) in (indicate the name of the organization) , paragraph 5 of Article 77 of the Labor Code of the Russian Federation." At a new place of work after indicating full name elected body, as well as the abbreviated name of the elected body (if any), in column 3 of the “Information about work” section of the work book, an entry is made about what job (position) the employee was elected to, and in column 4 the decision of the elected body, date and number of its acceptance.

7. Features of filling out a duplicate work book

7.1. A duplicate of the work book is filled out in accordance with sections 1 - 6 of these Instructions.
7.2. If an employee, before joining this organization (to to this employer) has already worked, then when filling out a duplicate work book in the section “Information about work” in column 3, first of all, an entry is made about the total and/or continuous work experience as an employee before joining this organization (to this employer), confirmed relevant documents. The total work experience is recorded in total, that is, the total number of years, months, days of work is indicated without specifying which employer, in what periods of time and in what positions the owner of the work book worked in the past. After this, the total and/or continuous work record length of service, confirmed by properly executed documents, is recorded for individual periods of work in the following order: column 2 indicates the date of employment; Column 3 records the name of the organization (employer) where the employee worked, as well as the structural unit and job (position), specialty, profession indicating the qualifications for which the employee was hired. If the submitted documents confirm that the employee was transferred to another permanent job in the same organization (with the same employer), then a corresponding entry is also made about this. Then in column 2 the date of dismissal (termination of the employment contract) is indicated, and in column 3 - the reason (ground) for dismissal, if the document submitted by the employee contains such data In the event that the documents do not fully contain the above information about work in the past, only the information available in the documents is entered into the duplicate work book. Column 4 indicates the name, date and number of the document on the basis of which the corresponding entries in the duplicate were made. Original documents confirming work experience, after making copies of them and duly certified by the employer or HR department are returned to their owner. The employer is obliged to assist the employee in obtaining documents confirming his work experience prior to joining the employer.

When there are changes in the employee's professional activities, appropriate entries must be made in the document confirming the length of service. The rules for filling out a work book by an employer are established in the Labor Code of the Russian Federation, and for 2015-2016 the samples remained unchanged.

Correctly filling out a work book: why is it needed and who makes the entries?

A work book is a document containing all the information about professional activity employee. Upon reaching the legal age, it is used to register and calculate a pension. The employer needs the books of his subordinates to calculate various payments, so it is very important to fill out everything correctly.

Regardless of what actions are taken in relation to an employee, changes are made in a company by an individual entrepreneur or in an enterprise by a specialist in the HR department, accounting department, or the director himself. Rules for filling out a work book in 2015-2016 and samples for individual entrepreneurs and legal entities are the same.

It is worth noting that an individual entrepreneur or LLC represented by a director should not establish this document for yourself: in Art. 66 of the Labor Code of the Russian Federation establishes that only employees can have a book, because The length of service of an entrepreneur is counted from the moment of registration with the Tax Service and thanks to cash receipts into the Pension Fund.

The book must be completed no later than a week from the first day of employment, and only if the employee has worked for 5 days or more, and this is his main job. If he only plans to combine, then entries are made at his request and on the basis of a document that is an official confirmation of his activities in another company.

  • Information about hiring.
  • Receiving a transfer to another company or to another position.
  • Change of full name in connection with certain events.
  • Serving in the ranks of the Russian Army, Ministry of Emergency Situations, Ministry of Internal Affairs, Federal Drug Control Service and others government agencies.
  • Advanced training or receiving awards.

Decree of the Government of the Russian Federation dated April 16. 2005 No. 225 established individual rules for making changes to books:

  • Any entries are made in black or blue ink in great detail; abbreviations are not allowed. The information is assigned a specific serial number according to the section.
  • The employee's passport details are indicated exclusively on the title page.
  • Further changes in the business process must be reflected no later than seven days from the date of issuance of the Director’s Order. If the employee resigns, then the information is entered on the same day, taking into account the text of the Order, as well as referring to the article of the Labor Code of the Russian Federation.

You cannot indicate in the document information about regular bonuses, which are usually paid every month.

Work book: sample filling when applying for a job

The title page must be completed first and upon presentation of the passport.

It is very easy to fill out the title page of the work book using the sample. To do this you need to enter certain data:

  • Last name, first name, patronymic. They can be viewed in your passport or other identity document.
  • Date of birth is indicated by numbers: for example, 10/02/1990.
  • Education. To confirm its availability, a diploma, certificate, or a certificate of unfinished training is provided if the employee is still a student.
  • Speciality. This refers to a profession acquired in an educational institution.

The column “Information about work” is an extract. Most often, employees ask to make a photocopy of several pages from it, and it contains the following information:

  • Date and number of entry in numbers.
  • Information about admission, dismissal, etc. For example: LLC "Mercury", accepted into the planning department for the position of economist.
  • What is the reason for such an entry: it is usually served by the publication of the Executive Order. Example: Order No. 762 dated 10/11/2015.

If a specialist decides to resign, then this is also recorded on the lines immediately following the previous information:

  • As before, the number is indicated in order, and the date of the last working day is indicated in Arabic numerals.
  • Reasons for dismissal: for example, if he leaves at his own request, it is necessary to refer to clause 3 of Art. 77 Labor Code of the Russian Federation.

Seal of the organization: it must be affixed by the director or specialist of the company authorized to use it. It is also necessary to put your signatures on the person who filled it out, and on the person resigning.

Filling out work books when transferring to another position in examples

The document, which is very necessary for a specialist, includes adjustments even when he was transferred to another position:

  • Mandatory entry number in order of priority.
  • Date of official translation in simple numbers.

Entering information about awards

Information about awards received is displayed in a number of cases: when the employee was thanked, a one-time bonus was issued, he was nominated for a title, or was awarded a valuable gift or certificate.

How to fill in the following data:

  • The entry is assigned a serial number, followed by the date it was made.
  • Under the line with the name of the company and the title of the position, an entry with approximately the following content is written: “gratitude was expressed for the development of an effective economic plan for the enterprise.”
  • Next, the Order and information about it are indicated: date and number.

Instructions for filling out work books when changing your last name

If the employee got married or there were other reasons for changing her last name, then all information is entered on the basis of the passport or other official documents she brought.

How the adjustment occurs:

  • On the title page, the previous surname is crossed out with a solid line, and the new surname is written there in the empty space.

If his book is lost or misplaced, the employee has every right to submit a written application to the employer or the personnel department from the last place of employment for the issuance of a duplicate; it is issued within 15 days. If the company is to blame for the loss, then all restoration responsibilities fall on it. To make a duplicate, the following documents are required:

  • Orders or their copies certified by management on hiring, dismissal, promotion, transfer.
  • Certificates from the State Archive in case the previous enterprise was liquidated.
  • Employment contracts and certificates of length of service from previous places of work.
  • Salary statements.
  • Personal accounts of a specialist in a company where it is necessary to confirm his work activities.
  • When the length of service has been restored in court, an appropriate decision is needed.

Between employees and employers there are labor relations. Cooperation between both parties is regulated by law. Partnership relations, rights and responsibilities in the sphere of labor, character work activity are documented.

Work book - what is it and what is it for?

A work book is a personal document that reflects the length of service, quality, duration of work at each place of employment, and information about the nature of a person’s activity.

Valid samples are from 1938, 1973, 2003, 2004; they fully participate in the working circulation and do not need to be replaced.

The document is required for:

  • actual display of work activity;
  • informing the employer about the reasons for the employee’s dismissal, the length of his service, awards, reprimands, etc.;
  • realization of rights to pension provision in the future.

Review of legislation

Legislative acts regulate the norms and rules in accordance with which the work book is drawn up, these include:

  • Labor Code of the Russian Federation. The provisions of the Labor Code of the Russian Federation clarify the concept of “work book”, define the employer’s responsibilities for maintaining and storing personal documents, and regulate the procedure and principles for their preparation. The Labor Code is a reference book for a good personnel officer.
  • Code of the Russian Federation ( Code of Administrative Offenses) about administrative offenses. The document contains standards that make it possible to determine the level of responsibility for non-compliance with the rules for handling work books. The inspector of the personnel department or personnel service is guided by the latest version of the law dated December 30. 2001 No. 195-FZ, as amended. dated June 23, 2014.
  • Government decree " About work books» dated 04/16/2003 No. 225 as amended. dated March 25, 2013 Approves the form and sample of the personal labor document, insert, regulates the maintenance and appropriate storage.

Rules for storing and maintaining work books

Maintaining

Dates

  • Entries of absolutely all dates are made strictly in Arabic numerals. Format XX – month, day; in format XXXX - year. If a person was hired for a position on July 10, 1990, the entry will be corresponding - “07/10/1990”;

Ink colors

  • Records are made carefully by the HR inspector using a fountain pen, ballpoint pen, rollerball pen, with ink that is resistant to light and humidity, purple, blue or black. Ink must be presented exclusively in paste form. The opinion about the permissibility of using gel pens is wrong;

Abbreviations are not allowed

  • Abbreviations in the work book are unacceptable. Records are entered accurately, correctly and in full;

Composition of information

The standards are regulated by the Labor Code:

  • information about the employee;
  • the nature of the work performed;
  • transfer to another place of permanent activity;
  • dismissal;
  • grounds for termination of the employment contract;
  • information about remunerations and incentives.

According to Article IV of the Labor Code, penalties are not included in the document, with the exception of precedents when the main disciplinary action becomes dismissal.

Language

  • state language of the Russian Federation;
  • state the language of the republic within the Russian Federation, if this language is officially established.

Compliance with the order

Based on the employer’s order, the following information is entered into the document:

  • about the activities performed;
  • about transfer to another type of activity/job;
  • about the employee’s qualifications;
  • about dismissal;
  • about the award.

Deadlines for depositing

  • the document must be completed and maintained for each employee employed in the organization for more than five days (with the exception of employers who are individuals), if this place of employment is the main one;
  • all data is entered no later than seven days, in case of dismissal - on the same day;

The responsible person - the HR department inspector - is obliged to meet the deadlines established by law.

Serial number

  • all entries are without abbreviations, within one section they have their own serial number;

Exceptions

  • when renaming an organization, columns ӀӀӀ and IV of the document must be filled out. In the section under the heading “Information about work”, in column III, the following entry is made: “Organization X has been renamed X since the date of XX.” The fourth column contains the grounds for renaming - an order, indicating the date and number;
  • the record of continuous service is not numbered when filling out a duplicate. According to the law, when working with a duplicate document, information about the employee’s continuous work experience before entering the current place of work is entered in the section under the heading “Information about work” in Column III. This is true if the person has already worked before joining this employer. This employment record is not numbered;
  • The names of employers are not subject to numbering (in the columns “Information about work books”, “Information about work”).

Familiarization of employees with records

  • the employer is obliged to familiarize the employee working in the organization with each record that relates to the nature of the activity performed, transfer to another position with permanent employment, or dismissal. The owner of the document must sign his personal card, where the entry made in the Labor Code is repeated;
  • when the employment contract has expired, all entries made in the document during the work activity in this organization are certified. The employer himself or the person responsible for office work and personnel can act as a witness. The document is stamped, signed by the manager, and signed by the employee;
  • After filling out the title, the employee signs on it in the appropriate line, thereby certifying the correctness of the information entered. Similarly, the signature is affixed to the title of the duplicate, insert.

Persons responsible for maintaining the document

  • The employer bears the full range of responsibility. The order or instruction of the employer provides grounds for the appointment authorized person, to whom all powers and responsibility for storage, maintenance, accounting and issuance of technical documentation are delegated. This person's signature authority allows all records to be certified;
  • these persons bear liability regulated by law;
  • When carrying out inspections, orders and instructions for the appointment of a responsible person are always checked by the State Labor Inspectorate.

Storage

The storage of work records must be organized in a specially designated place, inaccessible to third parties. In almost any organization it is a safe or an iron cabinet with a lock. A special tray or box should be provided for the books themselves.

If documents are damaged during storage, the organization may be subject to penalties. The storage period and accounting of work books is regulated by the following standard documents:

  • unclaimed TC – 75 years;
  • TC traffic record book – 75 years old. The book is filled out in the personnel department, by the personnel service, depending on the procedure established in the company. It contains data on TCs accepted for storage and issued;
  • receipt and expenditure book for accounting of TC forms – 5 years. The document takes into account the number of TCs and inserts for them. The book is issued by the accounting department and displays the consumption of these document forms with the number and series of each. To date standard document loses its relevance, since employees have the right to purchase work books with their own hands.

Books must be presented in a single, state-approved form. Upon receipt of the technical documentation, the employee leaves a signature on the personal card and logbook. Books are forms strict reporting, they are numbered, stitched, endorsed with the signature and seal of the employer.

How to fill out a work book correctly

Front page

  • According to the passport data, the employee’s full name is entered without abbreviations. If a change in surname occurs during work, the authorized person makes the correction on the basis of a passport and a certificate of change of surname. The previous data is crossed out with one line, the new data is added. The personnel officer writes on the cover a link to the document that served as the basis for the change, certifies the entry with a signature and seal. In some cases, instead of a passport, the personnel officer may be guided by another identification document.
  • Date of birth – the value is written according to the formula day.month.year.
  • Education. Data is entered based on the document provided by the employee, without abbreviations. If the employee has increased the level of education or received additional education, the data is replenished without crossing out.
  • The specialty is written strictly in the nominative case, on the basis of an education document - for example, “salesman”. If an employee enters a place of employment that requires special knowledge or special training, there must be additional documents, confirming this knowledge.
  • Date filled. After hiring, completion must be completed within five days.
  • Signature and transcript of the responsible person and employee.
  • Seal of the organization.

Information about hiring

  • Seal of the organization, details in words. If there is one, its abbreviated name is indicated.
  • I column. Serial number of the entry. If this is the first entry, put the number “1”, etc.
  • II column. The date the employee was hired.
  • Recruited/accepted into “department name” for position “X”. Information is entered in accordance with the staffing schedule. Fits on the same level as the date of admission in column III.

Transfer records

  • If a person is transferred from external organization, records of dismissal are made in column III “Information about work”, indicating information about the employee’s initiative (if any). Next is a record of admission to a new place of employment, with a note about the implementation of the transfer procedure.
  • For an internal transfer, it is sufficient to indicate the new place of employment and the basis for the transfer.

Algorithm for making transfer entries

  • Column I - entry number - the serial number is entered;
  • II column. The calendar date is indicated in Arabic numerals;
  • Column III – record of transfer;
  • Column IV – number and date of the document that served as the basis for the transfer.

Dismissal

Algorithm for making records of dismissal:

  • Column I is entered with the serial number of the entry;
  • Column II – the calendar date of dismissal is indicated in Arabic numerals;
  • Column III – record of dismissal with reference to Art. Labor Code (in full accordance with the wording of the law);
  • Column IV - No. and date of the document that served as the basis for dismissal.

Exceptions: if the employee does not appear to receive the document in hand, the employer sends him a notice of the need to appear and receive the technical documentation or provide consent to its sending by post. Starting from the date of filing the notice, the employer is relieved of responsibility for storing the document.

Part-time job

If the employee shows initiative, a record of part-time work is entered into a document in the personnel department of the main place of employment. The employee must bring the following documents to the HR department:

  • a copy of the employment agreement certified by the director of the part-time company;
  • request to make an entry in writing.

How to properly certify a copy

  • the authorized person makes copies of all pages of the document that contain entries;
  • the copy sheets are stamped with a stamp, the current date, a resolution “the copy is correct”, and the signature of the HR department inspector or other authorized person. The document must contain a note indicating that the employee is currently working in this position.

A copy of the work book is accepted at the place of request as a document confirming the person’s employment.

Duplicate

In the event that a document is lost due to the fault of an employee, he must notify the employer about this by filing a statement “about the loss of a work book.” Within 15 working days from the date of submission of the application, the employer provides a duplicate.

Information contained in the duplicate

  • the total length of service of the employee before entering this place of employment. The length of service must be indicated in summary, without specification;
  • specific information about activities in this organization.

Grounds for issuing a duplicate

  • TC is unsuitable for use;
  • loss of TC;
  • a court decision declaring an entry in the Labor Code invalid. The employer provides the employee with a duplicate containing all records except the unusable one.

Liner

An insert is required when it is not possible to make new entries in a document. This is a legal continuation of the Labor Code, which is issued and subject to registration in general procedure. The insert without labor is invalid. The book itself must contain a note indicating the attachment of an additional document.

Errors

  • On the title page, erroneous information is crossed out with one line, and the correct information is entered. On back side The cover must contain a link to the relevant document confirming the correctness of the new data.
  • Section corrections do not allow strikethroughs. The new entry is considered invalid, which must be registered. Then the specialist enters the correct information.
  • Errors are corrected only by the actual employer. If adjustments are made at a new place of work, there must be a basis in the form of a document from the previous employer who made the inaccuracy.
Algorithm for recording errors
  • Column I - serial number of the entry;
  • Column II - the date on which the entry was declared invalid, that is, the actual date of the corrections;
  • Column III verbatim: “The entry under No. X is invalid”;
  • Column II – the date of the new correct entry is indicated;
  • Column III – a new entry is made;
  • Column IV – number and date of the document that became the basis for entering the correct data.

Filling out a work book by an individual entrepreneur

An individual entrepreneur carries out personnel records according to the same scheme as all employers, but can use a simplified scheme.

Algorithm for recording employment records for individual entrepreneurs

  • Column III - the name of the employer is indicated - “ Individual entrepreneur XXX";
  • Column I – serial number of the entry;
  • Column II – date of admission to the staff;
  • Column III - at the same level as the date - information about hiring, indicating the position;
  • Column IV – number and date of the order, which is the basis for enrolling the employee on staff.

If the employer is an individual

In this case, the employer does not have the legal right to make entries in the labor code of employees. Individual, acting as an employer, does not have the right to issue a document to those employees who are starting work for the first time.

Additional instructions

  • Records of the appointment/election of the general director of the company: in the wording on the employment of a director of the company, you can use the word “elected” instead of “appointed” if the director is elected by the general meeting. Column IV must indicate the protocol general meeting, order, with corresponding numbers and dates.
  • Recruitment to a branch. If an employee is hired by a branch, in practice it is legal to use two options for indicating the branch: in the entry for the position itself and in the header, after indicating the name of the organization - both entries are kept in Column III.

Appendix No. 1. Instructions for filling out work books


This Instruction, in accordance with paragraph 13 of the Rules for maintaining and storing work books, producing work book forms and providing them to employers (hereinafter referred to as the Rules for maintaining work books), approved by Decree of the Government of the Russian Federation of April 16, 2003 N 225 “On work books”, establishes the procedure for filling out work books, inserts in them, duplicates of work books (hereinafter referred to as work books).

1. General provisions

1.1. Entries of dates in all sections of work books are made in Arabic numerals (day and month - two digits, year - four digits). For example, if an employee was hired on September 5, 2003, an entry is made in the work book: “09/05/2003”.

Entries are made carefully, using a fountain or gel pen, a rollerball pen (including a ballpoint pen), light-resistant ink (paste, gel) in black, blue or purple, and without any abbreviations. For example, it is not allowed to write "pr." instead of "order", "dispatch." instead of "order", "trans." instead of "translated" etc.

1.2. In the sections “Information about work” and “Information about awards” of the work book, crossing out previously made inaccurate, incorrect or other invalid entries is not allowed.

For example, if it is necessary to change a specific entry about employment in the “Job Information” section, after the corresponding last entry in this section, the subsequent serial number and the date the entry was made are indicated, in column 3 the entry is made: “The entry with such and such a number is invalid.” After this, the correct entry is made: “Accepted in such and such a profession (position)” and in column 4 the date and number of the order (instruction) or other decision of the employer, the entry from which was incorrectly entered into the work book, is repeated, or the date and number of the order is indicated ( order) or other decision of the employer, on the basis of which the correct entry is made.

In the same manner, a record of dismissal or transfer to another permanent job is invalidated if the dismissal or transfer is recognized as illegal by the employer, the control and supervisory body, the labor dispute resolution body or the court and reinstatement to the previous job or the wording of the reason for dismissal is changed. For example: “The entry for number so-and-so is invalid, reinstated to previous job.” If the wording of the reason for dismissal is changed, an entry is made: “The entry for number so-and-so is invalid, dismissed (new wording is indicated).” Column 4 makes reference to the order (instruction) or other decision of the employer on reinstatement or changing the wording of the reason for dismissal.

If there is an entry in the work book about dismissal or transfer to another permanent job, which is subsequently declared invalid, upon a written application from the employee, a duplicate work book is issued without making an entry declared invalid. In this case, in the upper right corner of the first page of the duplicate work book, the inscription: “Duplicate” is made. On the first page (title page) of the previous work book it is written: “Instead, a duplicate was issued,” indicating its series and number.

2. Filling out information about the employee

2.1. The information about the employee provided for by the Rules for maintaining work books, indicated on the first page (title page) of the work books, is filled out as follows:

last name, first name and patronymic are indicated in full, without abbreviation or replacement of first and middle names with initials, date of birth is recorded in full (day, month, year) on the basis of a passport or other identification document (for example, military ID, foreign passport, driver's license, etc. .);

registration of education (basic general, secondary general, primary vocational, secondary vocational, higher vocational and postgraduate vocational) is carried out only on the basis of properly certified documents (certificate, certificate, diploma, etc.);

a record of incomplete education at the appropriate level can be made on the basis of submitted duly certified documents (student card, grade book, certificate from an educational institution, etc.);

profession and/or specialty are indicated on the basis of documents on education, qualifications or the presence of special knowledge (when applying for a job that requires special knowledge or special training) or other properly executed documents.

2.2. After indicating the date of filling out the work book, the employee certifies the correctness of the entered information with his signature on the first page (title page) of the work book.

The first page (title page) of the work book is also signed by the person responsible for issuing work books, after which the seal of the organization (stamp of the personnel service) (if there is a seal) in which the work book was first filled out is affixed.

2.3. Changes to entries in work books about surname, name, patronymic and date of birth are made on the basis of a passport, birth certificate, marriage certificate, divorce, change of surname, name, patronymic and other documents and with reference to their number and date.

These changes are made on the first page (title page) of the work book. The previous surname or first name, patronymic, date of birth are crossed out with one line and new data is recorded. Links to the relevant documents are made on the inside cover of the work book and certified by the signature of the employer or a person specially authorized by him and the seal of the organization (or the seal of the personnel service) (if there is a seal).

2.4. Changes (additions) on the first page (title page) of the work book of entries about new education, profession, specialty obtained are carried out by adding existing entries (if they already exist) or filling out the corresponding lines without crossing out previously made entries.

3. Filling out job information

3.1. In column 3 of the “Work Information” section of the work book, the full name of the organization is indicated as a heading, as well as the abbreviated name of the organization (if any).

Under this heading in column 1 the serial number of the entry being made is indicated, in column 2 the date of employment is indicated.

In column 3, an entry is made about acceptance or appointment to a structural unit of the organization, indicating its specific name (if the condition of working in a specific structural unit is included in the employment contract as an essential one), the name of the position (job), specialty, profession indicating qualifications, and Column 4 contains the date and number of the order (instruction) or other decision of the employer, according to which the employee was hired. Records of the name of a position (job), specialty, profession indicating qualifications are made, as a rule, in accordance with the organization’s staffing table. If, in accordance with federal laws, the performance of work in certain positions, specialties or professions is associated with the provision of benefits or the presence of restrictions, then the names of these positions, specialties or professions and the qualification requirements for them must correspond to the names and requirements provided for in the relevant qualification reference books.

Changes and additions made in the prescribed manner to qualification reference books and the organization's staffing table are brought to the attention of employees, after which appropriate changes and additions are made to their work books on the basis of an order (instruction) or other decision of the employer.

If an employee is assigned a new rank (class, category, etc.) during the period of work, then a corresponding entry is made about this in the prescribed manner.

The establishment of a second and subsequent profession, specialty or other qualification for an employee is noted in the work book indicating the ranks, classes or other categories of these professions, specialties or skill levels. For example, a repairman was assigned the second profession “Electric and gas welder” with the assignment of the 3rd category. At the request of the employee, information about part-time work is recorded in the work book at the place of main work on the basis of a document confirming part-time work. In column 1 of the “Work Information” section of the work book, the serial number of the entry is entered, in column 2 the date of employment as a part-time worker is indicated, in column 3 an entry is made about acceptance or appointment as a part-time worker in a structural unit of the organization, indicating its specific name ( if the condition of working in a specific structural unit is included in the employment contract as an essential one), the name of the position, specialty, profession indicating qualifications, column 4 indicates the name of the document on the basis of which the entry was made with reference to its date and number. A record of dismissal from this job is made in the same manner.

3.2. If during the employee’s work the name of the organization changes, then an entry about this is made in a separate line in column 3 of the “Information about work” section of the work book: “Organization such and such has been renamed to such and such date,” and in column 4 it is entered the basis for renaming is an order (instruction) or other decision of the employer, its date and number.

3.3. In the work books of persons who have served a sentence of correctional labor, an entry about the non-inclusion of work time during the period of serving the sentence in the continuous work experience is made as follows. In the “Information about work” section of the work book, in column 1 the serial number of the entry is entered, in column 2 - the date of entry; in column 3 an entry is made: “Work time from such and such a date (day, month, year) to such and such a date (day, month, year) is not counted towards continuous work experience.” Column 4 indicates the basis for making an entry in the work book - an order (instruction) or other decision of the employer (issued in accordance with the court verdict), its date and number.

3.4. When continuous work experience is restored in accordance with the established procedure, the following entry is made in the employee’s work book at the last place of work in column 3 of the section “Information about work”: “Continuous work experience was restored from such and such a date, month, year”, in column 4 a reference is made to the corresponding name of the document on the basis of which the entry was made, with reference to its date and number.

4. Filling out information about the award

The procedure for entering information about awards is as follows: in column 3 of the “Information about awards” section of the work book, the full name of the organization is indicated as a heading, as well as the abbreviated name of the organization (if any); below in column 1 the serial number of the entry is entered (numbering increasing throughout the entire period of the employee’s work activity); Column 2 indicates the date of award; Column 3 records who awarded the employee, for what achievements and with what award; Column 4 indicates the name of the document on the basis of which the entry was made, with reference to its date and number.

5. Filling out information about dismissal (termination of employment contract)

5.1. An entry about dismissal (termination of an employment contract) in the employee’s work book is made in the following order: in column 1 the serial number of the entry is entered; Column 2 indicates the date of dismissal (termination of the employment contract); in column 3 an entry is made about the reason for dismissal (termination of the employment contract); Column 4 indicates the name of the document on the basis of which the entry was made - an order (instruction) or other decision of the employer, its date and number.

The date of dismissal (termination of an employment contract) is considered the last day of work, unless otherwise provided by federal law, an employment contract or an agreement between the employer and employee.

For example, when an employment contract with an employee is terminated due to a reduction in staff, October 10, 2003 is determined to be the last day of his work. The following entry must be made in the employee’s work book: in column 1 of the “Work Information” section, the serial number of the entry is entered, in column 2 the date of dismissal is indicated (10.10.2003), in column 3 the entry is made: “Dismissed due to a reduction in the number of employees of the organization, paragraph 2 ", column 4 indicates the date and number of the order (instruction) or other decision of the employer on dismissal.

5.2. Upon termination of an employment contract on the grounds provided for in Article 77 of the Labor Code of the Russian Federation (except for cases of termination of the employment contract at the initiative of the employer and due to circumstances beyond the control of the parties (clauses 4 and 10 of this article), an entry about dismissal is made in the work book ( termination of the employment contract) with reference to the relevant paragraph of this article.

For example: “Dismissed by agreement of the parties, paragraph 1” or “Dismissed at his own request, paragraph 3 of Article 77 of the Labor Code of the Russian Federation.”

5.3. When terminating an employment contract at the initiative of the employer, an entry about dismissal (termination of the employment contract) is made in the work book with reference to the relevant paragraph of Article 81 of the Labor Code of the Russian Federation or other grounds for terminating the employment contract at the initiative of the employer, provided for by law.

For example: “Dismissed due to the liquidation of the organization, paragraph 1 of Article 81 of the Labor Code of the Russian Federation” or “Dismissed due to termination of access to state secrets, paragraph 12 of Article 81 of the Labor Code of the Russian Federation.”

5.4. When an employment contract is terminated due to circumstances beyond the control of the parties, an entry is made in the work book about the grounds for termination of the employment contract with reference to the relevant paragraph.

For example: “Dismissed due to non-election to a position, paragraph 3 of Article 83 of the Labor Code of the Russian Federation” or “The employment contract was terminated due to the death of an employee, paragraph 6 of Article 83 of the Labor Code of the Russian Federation.”

5.5. When an employment contract is terminated on additional grounds provided for by the Labor Code of the Russian Federation or other federal laws, records of dismissal (termination of the employment contract) are made in the work book with reference to the relevant article of the Labor Code of the Russian Federation or other federal law.

For example: “Dismissed due to repeated gross violation of the charter of an educational institution within a year, paragraph 1 of Article 336 of the Labor Code of the Russian Federation” or “Dismissed due to reaching the age limit established for holding a public position in the civil service, paragraph 2 (1) of the article 25 of the Federal Law of July 31, 1995 N 119-FZ "On the Fundamentals of the Civil Service of the Russian Federation".

5.6. When an employment contract is terminated at the initiative of an employee for reasons that are associated by law with the provision of certain benefits and benefits, a record of dismissal (termination of the employment contract) is entered into the work record book indicating these reasons. For example: “Dismissed of her own free will due to the transfer of her husband to work in another locality, paragraph 3 of Article 77 of the Labor Code of the Russian Federation” or “Dismissed of her own free will due to the need to care for a child under the age of 14 years, paragraph 3 of the article 77 of the Labor Code of the Russian Federation."

6. Features of filling out information about dismissal (termination of an employment contract) and hiring (appointment) in connection with the transfer of an employee to another permanent job with another employer (to another organization) or his transfer to an elective job (position)

6.1. Upon dismissal (termination of an employment contract) in connection with the transfer of an employee to another permanent job with another employer (another organization), in column 3 of the “Information about work” section of the work book, it is indicated in what order the transfer is carried out: at the request of the employee or with his consent .

When hired to a new place of work, an entry is made in the employee’s work book in column 3 of the “Information about work” section, provided for in paragraph 3.1 of these Instructions, indicating that the employee was accepted (assigned) in the order of transfer.

6.2. Upon dismissal (termination of an employment contract) in connection with the transfer of an employee to an elective job (position) to another employer (to another organization), an entry is made in the work book: “Dismissed in connection with the transfer to an elective job (position) in (indicate the name of the organization) , paragraph 5 of Article 77 of the Labor Code of the Russian Federation."

At a new place of work, after indicating the full name of the elected body, as well as the abbreviated name of the elected body (if any), in column 3 of the “Work Information” section of the work book, an entry is made about what job (position) the employee was elected to, and in the column 4 indicates the decision of the elected body, the date and number of its adoption.

7. Features of filling out a duplicate work book

7.1. A duplicate of the work book is filled out in accordance with sections 1 - 6 of these Instructions.

7.2. If the employee had already worked before joining this organization (with this employer), then when filling out a duplicate work book in the section “Information about work” in column 3, first of all, a record is made about the total and/or continuous work experience as an employee before joining this organization (to this employer), confirmed by relevant documents.

The total work experience is recorded in total, that is, the total number of years, months, days of work is indicated without specifying which employer, in what periods of time and in what positions the owner of the work book worked in the past.

After this, the total and/or continuous work experience, confirmed by properly executed documents, is recorded for individual periods of work in the following order: column 2 indicates the date of employment; Column 3 records the name of the organization (employer) where the employee worked, as well as the structural unit and job (position), specialty, profession indicating the qualifications for which the employee was hired.

If the submitted documents confirm that the employee was transferred to another permanent job in the same organization (with the same employer), then a corresponding entry is also made about this.

Then, in column 2 the date of dismissal (termination of the employment contract) is indicated, and in column 3 - the reason (ground) for dismissal, if the document submitted by the employee contains such data.

In the event that the documents do not fully contain the above information about work in the past, only the information available in the documents is entered into the duplicate work book.

Column 4 indicates the name, date and number of the document on the basis of which the corresponding entries in the duplicate were made. The original documents confirming work experience, after making copies of them and duly certified by the employer or personnel service, are returned to their owner. The employer is obliged to assist the employee in obtaining documents confirming his work experience prior to joining the employer.
